Licensed Practical Nurse

Integrative Health Network LLC is currently hiring a contract position (starting at 1/4 FTE with the potential to increase to 3/4 FTE), seeking a dynamic LPN to join our expanding team in the areas of Southwest Washington & the Portland Metro area. This LPN will be responsible for providing oversight of client medication, distribution of said medication, and assists with coordination of care between primary care providers, supportive staff, private care agencies, and additional team members. 

This is a contracted position that requires you to have internet access, a work cell phone, and reliable transportation. This is a 1099 position. Pay rate depends on experience and readiness to perform the essential functions. Based on performance, after 90 day review, this position would ultimately lead to a part-time or full-time position. Essential Functions:

  • Follows all policies and procedures for medication administration, medication ordering, and delivery

  • Immediately reports med errors, or missing medications

  • Provides quality care to clients as indicated on the care plan

  • Promotes client independence in accordance with service plan

  • Demonstrates patience, compassion and integrity with clients

  • Completing monthly reports and documentation for each client

Additional Requirements:

  • Must possess special interest in, and a positive attitude about, working with those with mental health diagnoses and the elderly. 

  • Must have patience, tact, cheerful disposition, and enthusiasm as well as a willingness to handle difficult clients

  • Must possess the ability to make independent decisions when circumstances warrant such action

  • Must perform efficiently and effectively with little to no supervision

  • Maintains professional working relationships with all associates, vendors, etc.

  • Maintains confidentiality of all proprietary and/or confidential information

  • Attends client care conferences and assists in development of interdisciplinary plan of care for each client.

  • Attends daily, weekly, monthly meetings per agency policy and ensures follow up on any tasks in a timely manner to ensure client care needs are met.

  • Minimum of three (3) years experience as a Licensed Practical Nurse.

Job Requirements:

  • Active LPN License

  • CPR Certification 

  • BLS Certification

  • Active DL and auto insurance
